Please download, print, and email or submit to the school prior to absence. For our Time Credit guidelines, please click here.
Parent Report Sheet
Let us know how your child is doing! You may download and submit the Parent Report Sheet to the school at anytime, but especially during gradings. Remember that martial arts is more than kicking and punching--it is how you manage your life!
